Player Waiver/Release As player/parent/guardian/coach, I hold legal responsibility for the participant associated with this registration. Physical Capability Certification I certify and agree that the registered participant is physically capable of participating in the Unguarded Camps event.Conduct and Behavior As player/parent/guardian/coach, I acknowledge that I am responsible for ensuring the participant's conduct and behavior aligns with Unguarded Camps expectations. I understand that Unguarded Camps reserves the right to remove any participant from an event due to misconduct or failure to comply with instructions from event staff. Emergency Medical Treatment Authorization I hereby authorize Unguarded Camps, its employees, and volunteers to seek emergency medical assistance and treatment for the participant named in this application (“my child”) if the parents/guardians or emergency contact cannot be contacted. Release of Liability and Indemnification In consideration of the participant's participation in Unguarded Camps events, I release and agree to indemnify and hold harmless Unguarded Camps, and/or its affiliate organizations, employees, coaches, including but not limited to its owners, from any and all legal actions, causes of actions, claims, and demands for upon, or by reason of any injury which at any time may be sustained by the participant participating in an Unguarded Camps event. Photography Release I hereby authorize Unguarded Camps to take photographs of my child named in this registration (“my child”) during club activities, and to display and otherwise use these photographs without charge solely for the purpose of promotional material in connection with Unguarded Camps. Refund Policy By completing this registration form and payment, I understand that a refund will not be provided due to the participant (i.e., player/athlete) lack of participation, or failure to participate in or attend the Unguarded Camps event.
